Hair loss

What happens when you stop taking finasteride? Timeline, hair loss, and side effects

Finasteride is a treatment, not a cure - it only works while you use it. If you stop, the concentration of the medication in your body decreases within a few weeks and your body resumes DHT production¹. Hair loss returns within 6–12 months to the level it would have reached without treatment². Any side effects usually disappear within weeks³. Understanding what happens when you stop helps you make an informed decision about long-term use.

Why finasteride only works as long as you use it

Let’s start with the fundamental principle: finasteride is a treatment for hair loss, not a permanent solution. It only works as long as you use it - if you stop, the effect stops as well³.

Compare it to blood pressure medication or diabetes drugs. These treatments improve your health while you take them, but they don’t cure the underlying condition. The same applies to finasteride: it slows the hair-loss process, but it does not change the genetic predisposition behind it.

This isn’t a flaw of the medication - it’s simply how it works. Finasteride blocks the enzyme that converts testosterone into DHT, the hormone that drives hair loss. Once you stop taking it, that process resumes¹.

Why is this important? Because it creates realistic expectations. Finasteride is a long-term commitment. For many men that’s no issue - one pill a day is a small price for keeping their hair. For others, that commitment can be a reason not to start, or to stop at some point.

Both choices are completely valid. The key is making an informed decision with all the facts on the table.

What happens in your body when you stop with finasteride?

If you decide to stop finasteride, a clear biological timeline begins.

Within 5–6 hours: half-life reached
Finasteride has a half-life of about 5–6 hours in men under 60¹. This means that after 5–6 hours, half of the active substance has disappeared from your bloodstream. After 24–48 hours, most of the medication is out of your system.

Within 1–2 weeks: DHT production resumes
Once finasteride is out of your body, the enzyme 5-alpha-reductase starts functioning normally again. Your body begins converting testosterone into DHT again. Within a few weeks, your DHT levels return to what they were before treatment¹.

After 3–4 weeks: full DHT recovery
After about a month, DHT levels in the scalp are fully restored. This means hair follicles are once again exposed to the hormone that causes them to shrink².

The good news: no withdrawal symptoms

Unlike some medications, stopping finasteride does not cause withdrawal symptoms. You also don’t need to taper off - you can stop whenever you want³. There is no rebound effect where hair loss accelerates temporarily.

How quickly does hair loss return?

The question most men care about: how fast do you lose your results?

The timeline: 6–12 months
Studies show that hair loss returns within 6 to 12 months after stopping, reaching the level it would have been without treatment². This is a gradual process, not a sudden change.

How it typically unfolds

First 3 months:

you’ll likely notice little. DHT levels may be restored, but visible effects on hair take time.

Months 3–6:

hair loss gradually becomes noticeable. You may see thinning and the return of bald spots.

Months 6–12:

the process is largely complete. Your hair returns to where it would be without finasteride - including any additional progression that would have occurred in the meantime².

You lose all “gains”
This is important to understand: you don’t only lose the regrowth you achieved with finasteride - your hair loss also continues progressing. For example, if you used finasteride for three years, after stopping your hair doesn’t revert to where it was three years ago. It moves to where it would be today without those three years of treatment.

The hair cycle matters
Hair follicles go through cycles of growth, rest, and shedding. That’s why the process is gradual. Not all hairs are affected at once, but step-by-step over several months.

Do finasteride side effects go away when you stop?

For the small group of men who experienced side effects, this is often the most important question.

The general picture: yes, in almost all cases

If you were among the roughly 2–4% who experienced side effects during use, the good news is that for the vast majority, side effects resolve within a few weeks to three months after stopping³. Once the medication has cleared and hormone levels normalize, things typically fully return to baseline.

Recovery timeline (for those who had side effects)

  • Within 1–2 weeks: many men already notice improvement, because finasteride leaves the body quickly¹.

  • After 4–8 weeks: most men are fully back to their pre-finasteride baseline³.

A small caveat: post-finasteride syndrome

A very small number of men report persistent symptoms after stopping. This is referred to as post-finasteride syndrome and is characterized by side effects lasting longer than three months⁴. It’s important to emphasize that this is rare, and the scientific consensus is that in the overwhelming majority of cases, side effects fully resolve⁴.

When should you consult a doctor?

If you continue to experience side effects for more than three months after stopping, consult a doctor. The same applies to any unexpected symptoms. At Menkind, our doctors can support you through this process.

Why do people stop using finasteride? (or consider stopping)

Men stop finasteride for many reasons - each valid and personal. Common reasons include:

Side effects
For some men, sexual side effects - even if rare - outweigh the benefit of keeping their hair. That’s a completely reasonable trade-off.

Cost
Finasteride is a long-term investment. For some men, monthly costs no longer fit their budget at a certain point.

The hassle of daily use
Taking a pill every day requires discipline. Some men find it burdensome over time or forget regularly.

You’ve reached your desired result
Maybe your hair has stabilized and you’re satisfied. Some men then want to experiment with stopping to see whether it’s still necessary (spoiler: usually it is).

Life changes
Most men choose to temporarily stop finasteride when actively trying to conceive. Although effects on fertility are very rare and reversible upon discontinuation⁵, doctors recommend discontinuing finasteride use at least 12 weeks before planned conception as a precautionary measure. Since finasteride is completely eliminated from your system within a few weeks¹, sperm quality typically recovers quickly. Always discuss this with your doctor for personalized advice.

Or you may simply decide to accept hair loss as part of aging.

Considering alternatives
You may want to switch to another treatment such as minoxidil, or try a combination approach.

No judgment - just information
Whatever your reason: it’s your choice. The goal of this article isn’t to persuade you to continue or stop, but to explain what happens so you can make an informed decision.

What alternatives for finasteride are there if you want to stop?

If you’re considering stopping finasteride, there are several paths you can take.

Option 1: Stop completely and accept it
The most straightforward choice. You accept hair loss and embrace the change. For many men this feels freeing - no daily pills, no commitment, no costs.

Option 2: Switch to minoxidil
Minoxidil works through a completely different mechanism than finasteride. It stimulates blood flow to hair follicles instead of blocking DHT.
Pros: different side effects (mainly local skin irritation rather than sexual effects).
Cons: must be applied twice daily⁵.

Option 3: The “overlap method”
A smart strategy if you want to switch treatments without losing all progress. Start minoxidil before you stop finasteride. Use both together for a few months, then stop finasteride. This gives minoxidil time to start working before the finasteride effect fades⁵.

Option 4: Consider combination therapy
Research shows that combining finasteride and minoxidil is more effective than either alone⁵. If you currently use finasteride only and want to optimize results, you can add minoxidil. If you later stop finasteride, the minoxidil effect remains.

Option 5: Hair transplant as a more lasting option
For men seeking a more permanent solution, a hair transplant may be an option. It doesn’t fully “solve” the problem (you can still lose more hair), but outcomes can be stabilized with ongoing treatments.

The importance of medical advice
Whatever route you choose, discuss it with a doctor. The doctors at Menkind can guide you through the decision and help create a plan that fits your situation and goals.

Conclusion: stopping with finasteride in short

Finasteride only works while you use it. If you stop, hair loss typically returns within 6–12 months to the level it would have reached without treatment². For the vast majority of men, any side effects resolve within a few weeks³.

This makes finasteride a commitment - not a one-time fix, but a long-term treatment. For many men, that’s acceptable and easy: one pill a day to maintain their hair. For others, the commitment may become too burdensome over time.

Both perspectives are completely valid. What matters most is that you make an informed choice. If you decide to stop, you now know what to expect. If you decide to continue, you understand the commitment.

And remember: stopping doesn’t mean the end of your options. From minoxidil to combination therapy to acceptance - there are multiple paths forward. The doctors at Menkind are ready to support you, whichever direction you choose.
